Instructions: Print and complete form*
which will accompany sample(s) to be submitted. Each sample submitted
must include the signature of an authorized ordering person and a diagnostic
indication for the requested cytogenetic study. Follow this link for specific
specimen requirements.

Keep
all specimens at room temperature and deliver to the laboratory as soon
as possible.

Do
not fix samples in any manner contrary to maintenance of cell viability
(see Specimen Collection/Handling Requirements).

All
samples should be packaged to protect from temperature extremes (heat
and cold) and should be shipped at room temperature. Cool packs are reccommended during the hot summer months. (DO NOT FREEZE or SHIP ON ICE).

All
samples submitted should include an appropriate requistion form that
includes the authorized person ordering the test(s) and a working diagnosis
(see Cytogenetic Requistion Forms)

| All
specimens should be labelled and packaged as "Exempt Human Specimen" and should include a double container (e.g. a box within
a plastic ziploc bag) with enough absorbent material to contain the sample
should breakage or spilling occur (packaging materials are available
from the laboratory).

| Arrangements
for pickup of local samples may be made with the laboratory by calling
(352) 265-9900 during regular working hours.

| In
house (UF/Shands) and local samples may also be delivered to the Shands Clinical
laboratory.
|
All
samples originating outside of the Gainesville area should be delivered
by same day or overnight delivery (we recommend FedEx) to

- Monday
- Friday 8:00 AM to 5:00 PM
- Saturday 8:00 AM to 4:30 PM.
- Closed on Sunday

Prior notitice ensures a quality measure of expedient
tracking and procurement of samples (by our staff) that are not delivered
as expected.
