Workstation Registration for Access to HSC network
or to install UF Owned Software

This form is used to register a workstation for access to the HSC network if that workstation was not originally managed by the Dept of Pathology Computer Support Section. Generally this will be for a personally owned portable or vendor provided instrument interfaces connecting either directly or via wireless. This form is also required for any non-UF owned workstation on which UF owned software will be installed (required for nearly all UF software licenses). For all requests involving non-Pathology managed computers, a Pathology Faculty Sponsor is required.
